Submission Forms

Need to create a customer survey, employment application, or event nomination? You can create your own forms with an unlimited number of custom fields, choosing from field types such as text fields, drop-down menus, radio buttons, and file uploads.  When creating a new submission form, you will be able to specify the title, contact email address, introductory message shown above the form, and confirmation message shown when the form is submitted. When a visitor submits the form on the front end website, the email address(es) specified when creating the form will receive an instant email notification.
